How to Apply
1: Download and complete the official application Performa (Annexure A)
2: General and OBC candidates prepare Demand Draft of Rs. 500 in favor of ‘ESI Fund Account Number 1’ payable at Noida
3: Compile self-attested photocopies of MBBS and PG Degree/Diploma certificates with mark sheets
4: Ensure valid DMC/UPMCI Registration Certificate with PG qualification
5: Carry proof of identity and age (Matriculation certificate, Aadhar Card, PAN Card)
6: Carry latest Caste or EWS certificate if applicable (not older than one year for OBC/EWS)
7: Report to Conference Room, Ground Floor, ESIC Medical College and Hospital, Noida by 09:00 AM on interview date with all originals and application set
8: Government employees must produce No Objection Certificate (NOC)
