1: Fill out the prescribed Bio-data form with personal and service details
2: Affix recent passport-size photograph on the form
3: Attach self-attested educational certificates from SSC through Graduation
4: Enclose copy of Retirement Certificate or Retirement Identity Card
5: Provide copy of Pension Payment Book and ACRs for last five years
6: Include copy of valid photo ID such as Aadhaar or PAN Card
7: Include two additional photocopies of completed Bio-data
