- Download the application format (Prapatr-1 and Prapatr-2) from the Civil Court Sheohar official website (https://sheohar.dcourts.gov.in/) or obtain a copy from the office.
- Fill out the prescribed application form completely in your own handwriting.
- Attach self-attested photocopies of educational and other qualification certificates.
- Affix a passport-size photograph with self-attestation on the application form.
- Enclose a self-addressed envelope affixed with a registered postal stamp and write your contact number and email ID on the form.
- Send the completed application form via registered post to reach the Secretary, District Legal Services Authority, A.D.R. Bhawan, Sheohar on or before 30 November 2026 by 05:00 PM. Applications may also be submitted in person at the office between 10:00 AM and 05:00 PM.
- Ensure all required documents and postage are included; incomplete or late applications will be rejected.
